AzHealthFail exists to hold Arizona's health-regulatory and political apparatus — the Department of Health Services, the Governor's office, and the Legislature — accountable for failures in oversight, enforcement, and policy that let healthcare-sector harms occur or go unaddressed. It consolidates scattered accountability work into one durable, cross-referenced public record, and gives future cases a home instead of a one-off site each time.

This is a noncommercial project. It sells nothing, solicits no donations, and runs no advertising. It is published in the public interest, as commentary and documentation on the public conduct of public agencies and officials.

Because this site names sitting officials and evaluates their record, we hold to a hard editorial standard: every documented claim is tied to a checkable source, and opinion is labeled and kept structurally separate from fact. Some cases here arose from harm to real patients, including members of the author's own family. Out of respect for privacy, public copy keeps individual patients de-identified — referred to, for example, as "an established patient" — while formal complaints and filings submitted to agencies may be more specific as the process requires. We do not publish other people's private medical details.

Some matters documented here are the subject of active complaints or proceedings before agencies such as ADHS, AHCCCS, CMS, or Kepro. This site is informational: it records what was filed, what was answered, and what wasn't. It is not a party's brief, and nothing here should be read as a statement on behalf of any agency or tribunal.
